Billing

Please call before your appointment if you have any concerns about insurance coverage.

All co-insurances, co-pays, and deductibles will be expected at the time of service unless you have made other arrangements.

Cache Valley Women’s Center accepts cash, checks, Visa, Mastercard, Discover, and American Express.

We work with most major insurance companies and will submit your claims for you. We also accept all types of Medicaid, except for Molina. You may want to call your insurance company to verify that we work with your plan if you are unsure of coverage.

It is our policy that if your insurance carrier has not responded to your claim within 90 days, we reserve the right to transfer all associated liability to the patient or the guarantor. If we do not participate with your insurance plan, we will file your claim for you. However, you will be expected to pay at the time of service.

Please note the Cache Valley Women’s Center at the Lodge outsources pap smears, blood work, and pathology. You will receive a separate bill for these services. Please call the appropriate provider of these services if you have questions about their billing.
